Requesting Your Records
1. Submit a written request using one of the following methods:
- Through your secure client portal
- By email to: info@meadowsrgv.com
- By completing a records request form (available upon request)
2. Include the following information:
- Full name
- Date of birth
- Dates of service (if known)
- Description of records requested
- Where you would like the records sent
3. Provide a valid form of identification.
4. Submit your request to our office.
Processing Time: Requests are typically processed within 15 business days.
Fees: Reasonable fees may apply in accordance with Texas law.
